The Concern
Understanding dark circles under the eyes
- Structural (shadow): volume loss in the tear trough with age, genetics or weight loss; puffiness above a hollow deepens the shadow.
- Pigmented: excess melanin from genetics, sun, rubbing (allergies, eczema) or post-inflammatory change; more common in medium and deeper skin tones.
- Vascular: thin, translucent lower-lid skin showing vessels; worse with fatigue, allergies, congestion, alcohol and smoking.
- Most patients have a mix; a simple test (gently stretching the skin or looking in different light) helps the provider classify it.
Treatment Options
How we treat it
1. Tear-trough dermal filler
- What it does for this concern
- Hyaluronic acid filler placed by an experienced injector to soften the hollow and the shadow it casts; immediate result, 24–48 hours of possible bruising/swelling; typically lasts 9–12+ months. Not for every anatomy (puffy bags or very thin skin may be unsuitable).
2. PRP (platelet-rich plasma) under-eye
- What it does for this concern
- Your own platelet concentrate injected or microneedled to support skin thickness and tone; series of 3, 4–6 weeks apart; 1–3 days of mild swelling; suits thin, crepey or slightly discolored skin.
3. Pico laser (low-fluence)
- What it does for this concern
- Gentle pigment clearance for brown under-eye discoloration with minimal heat; 4–6 sessions; minimal downtime; used across skin tones with eye protection.
4. Gentle chemical peels
- What it does for this concern
- Low-strength lactic/mandelic or brightening peels formulated for the periorbital area; series of 3–4; 1–2 days light flaking.
5. Prescription topicals
- What it does for this concern
- When appropriate, prescribed by a licensed provider after consultation: low-strength retinoid, azelaic acid or short, supervised brightener courses for pigmented circles; plus caffeine/vitamin C/niacinamide skincare.
Select cases: SpectraionX or Morpheus8 (eye-safe settings) for crepey under-eye skin; Sculptra is not used in the under-eye area.

Pricing: Pricing confirmed at consultation
Circles caused by a hollow, pigment or thin skin are structural and do not resolve with rest. The consultation identifies which type you have.
In experienced hands with appropriate anatomy it is a common treatment; risks include bruising, swelling and, rarely, vascular complications, which your provider reviews with you.
Low-heat Pico and brightening peels can lighten pigment over a series; sun protection and topicals help maintain.
Puffiness is handled differently (sometimes a surgical referral); filler can worsen bags, so classification matters.
Often 9–12 months or longer, because the area moves little; touch-ups are planned at consultation.
